| dc.description.abstract | Předkládaná bakalářská práce si klade za cíl analyzovat problematiku ucelené rehabilitace jako významné součásti terapie klientů na Oddělení následné péče v Českých Budějovicích.
Ucelená rehabilitace se stává nepostradatelnou složkou naší společnosti. Ucelenost v rehabilitaci znamená především včasnost, návaznost jednotlivých složek na sebe a také komplexnost. Jejím smyslem a cílem je co nejúplnější začlenění lidí se zdravotním postižením do aktivního života. Klasifikace dle ICF přináší podstatnou změnu; nehodnotí pouze poruchy a funkce, ale zároveň i sociální vlivy a faktory prostředí V návaznosti na klasifikaci ICF byl vytvořen strategický plán péče ve smyslu ucelené rehabilitace.
Bakalářská práce je rozdělena na část teoretickou a praktickou. Teoretická část práce ve čtyřech kapitolách pojednává o současném stavu problému, definuje stárnutí a stáří, charakterizuje složky ucelené rehabilitace, pojetí a cíle Mezinárodní klasifikace funkčnosti, postižení a zdraví (ICF).
Praktická část bakalářské práce předkládá empirický výzkum realizovaný s cílem analyzovat možnosti ucelené rehabilitace na Oddělení následné péče v Českých Budějovicích. Výzkumný soubor byl tvořen pěti vybranými seniory, kteří byli vybráni na základě diagnóz, místa a způsobu léčby a s ohledem na rozsah péče a služeb, které v průběhu léčby využili. Součástí je také kapitola, která nabízí na základě vyhodnocení výsledků výzkumu některé návrhy a opatření vhodné pro praxi. | cze |

| dc.description.abstract-translated | The submitted bachelor thesis seeks to analyze the issue of comprehensive rehabilitation as a significant part of the therapy of clients at the department of follow-up care (hereinafter DFC) in České Budějovice.
The comprehensive rehabilitation is becoming an indispensable component of the care provided by our company. The comprehensiveness in rehabilitation means primarily timeliness, mutual links between it individual components and also completeness of the provided care. The purpose and the objective is the maximum possible integration of people with health handicaps into the active life with the expected quality. The comprehensive rehabilitation also includes a pro-active approach of the handicapped, who strives to participate in regular life.
The current situation is characterized with an unclear definition of roles of the individual members of the multidisciplinary team, insufficient communication and subsequently also lack of teamwork. Also missing is a detailed breakdown of the issues of comprehensive care and a strategy to resolve the client´s current situation in order to improve the quality of individual components of his/her life in the long-term time horizon.
The classification according to the International Classification of Diseases (ICD) focuses mainly on determination of a diagnosis but it lacks a detailed analysis of limitations and consequences which accompany the particular diagnosis.
The International Classification of Functioning, Disability and Health (ICF) has brought a substantial change; it does not evaluate only disorders and functions but also the social effects and factors of the environment. In connection with ICF a strategic plan of care has been developed in the sense of comprehensive rehabilitation. The comprehensive rehabilitation according to this concept contains all components and instruments aimed at a reduction of the pressure exerted by obstacles resulting from the health condition and seeks the maximum level of integration of the individual into the society. The current concept of rehabilitation is no more perceived as an exclusively healthcare activity in the sense of physiotherapy but as a interdisciplinary field, which includes not only healthcare but also social-legal and pedagogical-psychological support.
The theoretical part of the thesis deals in five chapters with the current status of the issue, it defines the term of aging and old age, the most frequent diseases in the senior age and characterizes components of comprehensive rehabilitation and their potential use for the individual diseases and the concept and goals of the International Classification of Functioning, Disability and Health (ICF)
The practical part of the thesis presents an empirical research with the objective to analyze possibilities of comprehensive rehabilitation at the department of follow-up care. The research sample consisted of five senior citizens selected based on their diagnoses, treatment methods and the scope of the care and services they used in the course of their treatment. The thesis also contains a chapter with some practical proposals and measures based on the research results. | eng |

Obhajoba:
Student představil práci adekvátním způsobem. Členové komise se ztotožňují s posudkem vedoucího práce. Autor se dokázal orientovat v problematice ucelené rehabilitace. Odpověděl na otázku člena komise, která se týkala funkcí multidisciplinárního týmu v Nemocnici České Budějovice, a. s. Vysvětlil z jakého důvodu využívá v práce pojem "handicap".
