| dc.description.abstract | Ischemická choroba dolních končetin postihuje dlouhodobě stále více populace. Včasná detekce a úspěšná léčba těsně koreluje s kvalitou a délkou života. Pokrok v rozvoji endovaskulárních technik je v posledních letech závratný. V řadě případů je průkaz o jejich efektivitě a bezpečnosti podáván ve chvíli, kdy se jejich indikace opouští. A místo nich nastupují novější modifikace používaných nástrojů, pomůcek a postupů. Díky tomu se snižuje invazivita výkonů a léčba je tak dostupnější stále více pacientům.
Endovaskulární intervence je poměrně ekonomicky náročný výkon. Tato myšlenka mne vedla vytvořit výzkum jako porovnání nákladů oproti chirurgické operaci. Pro porovnání ekonomické náročnosti jednotlivých výkonů jsem náhodně vybral soubor 5 pacientů, u kterých byla provedena perkutánní transluminální angioplastika na Radiodiagnostickém oddělení nemocnice České Budějovice a.s. Údaje o cenách použitého materiálu a léčiv jsem zpracoval a porovnal opět se souborem 5 náhodně vybraných pacientů, kteří podstoupili chirurgickou operaci cév na chirurgickém oddělení.
Výsledky ukazují, že angioplastika je jednoznačně nákladnější výkon. Celková cena záleží na více faktorech. U angioplastiky jde především o použití nejrůznějších nástrojů nebo stentů, v závislosti na rozsahu postižení tepen. Cena chirurgické operace se odvíjí hlavně od toho, zda je možné použít pacientovu žílu - vénu saphénu jako bypass, která je samozřejmě zadarmo. Pokud je nutné použít přímou náhradu, v podobě umělé cévní protézy, bylo zjištěno, že se standardně používá nejlevnější typ tzv. pletené cévní protézy v několika velikostech. Jen v případě možných komplikací v podobě zánětlivé infekce se používá výjimečně tzv. PTFE litá protéza.
Výsledky perkutánní transluminální angioplastiky potvrzují vysokou primární i sekundární průchodnost intervencí na ilických tepnách, která se při užití stentu vyrovná chirurgické revaskularizaci. Nejčastější úsek, kde jsou prováděny intervence je femororo-popliteální, kde je průchodnost nižší než u femoro-popliteálního bypassu. Ovšem díky nízké invazivitě je angioplastika méně rizikový výkon. | cze |

| dc.description.abstract-translated | The part of the population affected by the ischemic disease of lower extremities has been growing in the long-term horizon. The detection and the successful treatment correlates closely with the quality and length of life. The progress in the development of endovascular technologies is immense in the last years. In a number of cases the proof of their effectiveness and safety is provided in the moment when their indication is abandoned. Instead of them, newer modifications of instruments, auxiliary means and procedures are applied. Thanks to this, the invasive character of surgeries decreases and the treatment is more available to more and more patients than before.
The endovascular intervention is a quite demanding surgery from the economic point of view. This idea brought me to concept of research comparing the cost with the cost of surgical operation. To compare the economic demands of individual interventions I have chosen at random the group of 5 patients, having undergone the percutaneous trans-luminal angio-plastics in the Radio-diagnostic Ward of the Hospital in České Budějovice a.s.. The data of prices of the applied material and drugs were processed and compared again with the group of 5 randomly selected patients, who underwent the surgical operation of vessels in the surgical department.
The results show that angioplasty is a clearly more expensive intervention. The total prices depends on more factors. In case of angioplasty the concern is first of all the use of various tools or stents depending on the extent of affecting arteries. The price of the surgical operation depends mainly on the fact if it is possible to use the patient´s vein - vena saphena as bypass which is, naturally, free of charge. If it is necessary to apply the direct substitution in the form of artificial venous prosthesis it was found out that normally the most cheap type of the so-called woven venous prosthesis in several sizes is used. Only in case of possible complications in the form of inflammatory infection exceptionally the so-called PTFE cast prosthesis is used.
The results of percutaneous trans-luminal angioplasty confirm the high primary as well as secondary patency (passability) by intervention on iliac arteries, which is equal to surgical revascularization while using a stent. The most frequent section where the interventions are made, is femoro-popliteal section, where the patency is lower than in case of femoro-popliteal bypass. Thanks to the low invasive character of surgery, angioplasty is a less risky intervention. | eng |
