dc.description.abstract | Tato bakalářská práce je zaměřena na problematiku stavů spojených se vznikem
kompartment syndromu v přednemocniční péči. Tento stav se vyznačuje nárůstem tlaku
v uzavřeném anatomickém prostoru, který vede k poruše cirkulace a nervového čití
v daném okrsku. V přednemocniční péči se můžeme s kompartment syndromem setkat
při frakturách, popáleninách, alergických reakcích, traumatech způsobených úrazem,
elektrotraumatem. Dále pak u crush syndromu, dopravních nehod, kraniocerebrálních
poraněních a dalších stavů, jejichž průvodním znakem jsou otoky. Tato práce se
orientuje především na popáleniny, fraktury, elektrotraumata, nitrobřišní hypertenzi
a stavy způsobené hadím uštknutím. Nešetrná, či nevhodná manipulace s takto
postiženým pacientem, může zhoršit jeho následný stav, který může vést k poškození
pacienta až k fatálním důsledkům. Záměrem práce bylo zmapování teoretických
a praktických dovedností nelékařských zdravotních pracovníků, pracujících
v přednemocniční péči, v ošetření stavů, jejichž péče může rozvoj kompartment
syndromu oddálit, či naopak urychlit.
Sběr dat do výzkumu byl uskutečněn pomocí dotazníků a polostandardizovaných
rozhovorů. Na vyplnění dotazníků se podílelo dvacet náhodně vybraných respondentů.
Rozhovory byly uskutečněny s dvanácti respondenty. Vyhodnocení správných odpovědí
je uvedeno v tabulkách v praktické části této práce. Přepis rozhovorů je též součástí
praktické části, kde jsou barevně zvýrazněné správné a nesprávné informace, jež byly
řečeny dotazovanými zdravotnickými záchranáři. Vzorový test a soupis kladených
otázek i s jejich podotázkami jsou k nalezení v příloze 1 a 2. Obě složky výzkumu byly
zrealizovány se zdravotnickými záchranáři a všeobecnými sestrami pracujícími na
zdravotnických záchranných službách Jihočeského kraje a Kraje Vysočina.
Výzkumem bylo zjištěno, že pojem kompartment syndrom zdravotničtí záchranáři
a všeobecné sestry pracující na zdravotnických záchranných službách znají, ale
neuvědomují si jeho souvislost s přednemocniční péčí. Odpovědi respondentů se
v problematice ošetření akutních stavů souvisejících s tímto jevem v některých
případech silně lišily od doporučených postupů v použité literatuře. Dále se výzkum
zabýval typem a množstvím používaných roztoků v přednemocniční péči. I tato otázka
odhalila zajímavá zjištění. V poslední řadě byl výzkum orientován na možnost výkonu
akutní fasciotomie při cirkulárních popáleninách krku v přednemocniční péči. Tento
výkon byl respondentům interpretován jako život zachraňující, neboť při tomto stavu je
pacient vysoce ohrožen na životě a rychlý transport bez provedení uvolňujících nářezů
je pro něj vysoce rizikovým. Jedná se tedy o výkon, který by zdravotničtí záchranáři
měli znát alespoň po teoretické stránce a vnímat ho jako život zachraňující výkon,
i když k němuž nemají prozatím dostatečné kompetence. Respondenti během výzkumu
projevili zájem o tuto problematiku a snahu o prohloubení svých znalostí o tomto
výkonu.
Z výzkumného šetření vyplynulo, že zdravotničtí záchranáři a všeobecné sestry
pracující na zdravotnických záchranných službách mají celkové nedostatky ve
znalostech týkajících se této problematiky. Podle výzkumu a současně i jejich vyjádření
při rozhovorech, je důvodem jejich neúplných znalostí nedostatečná informovanost
v jednotlivých klinických oborech. Sami záchranáři poukázali na nedostatečné znalosti
Zlepšením této situace by mohlo být dostatečné proškolování v oborech
traumatologie, chirurgie a intenzivní péče nejenom zdravotnických záchranářů
pracujících na zdravotnických záchranných službách, ale i studentů tohoto oboru. Tato
práce by mohla sloužit jako námět pro hlubší prozkoumání tohoto prozatím dostatečně
neznámého tématu. Dále pak pro rozvoj odborných znalostí zdravotnických záchranářů
v ošetřování edematózních stavů a zkvalitnění péče o pacienty jakož to i jejich následné

dc.description.abstract-translated | This bachelor thesis focuses on conditions associated with compartment syndrome
development in prehospital care. Pressure increase in a closed anatomical space is
characteristic for this condition, leading to circulation and neural sensitivity affection in
the specified area. In the prehospital care, we may meet with the compartment
syndrome in fractures, burns, allergic reactions, traumas caused by an accident,
electrotraumas; further in crush syndrome, traffic accidents, craniocerebral injuries and
other conditions associated with swelling. This thesis focuses on burns, fractures,
electrotraumas, intra-abdominal hypertension and conditions caused by snake bite.
Inconsiderate or inadequate manipulation with affected patient may worsen his/her
condition and lead to deterioration of his/her health condition or even fatal
consequences. The aim of this thesis was to map the theoretical and practical knowledge
of non-medical health care professionals working in prehospital care, in treatment of
conditions able to delay compartment syndrome development or even to accelerate it.
Data collection for the research was performed using questionnaires and semistandardised
interviews. Questionnaires were filled by 20 randomly chosen respondents.
12 respondents were interviewed. Correct answers evaluation is listed in the tables in
the practical part of this thesis. Interviews transcript is a part of the practical part as
well, with colour coded correct and incorrect information provided by the interviewed
medical rescuers. A sample test and list of posed questions with sub-questions are
included in the attachment. Both parts of the research were performed with medical
rescuers and general medicine nurses working at medical rescue services of southbohemian
region and the region of Vysočina.
The research found that the term of compartment syndrome is well known by
medical rescuers and general nurses working at medical rescue services. However, they
are not aware of its association with prehospital care. In some cases, the answers of the
responders differed significantly, as for treatment of acute conditions associated with
this problem, from the recommended procedures according to the literature.
Furthermore, the research analysed the type and amount of used fluids in pre-hospital
care. This question showed interesting findings as well. Lastly, the research analysed
the options of acute fasciotomy in circular neck burns in prehospital care. The
respondents interpreted this procedure as life-saving, as this condition is life-threatening
for patients and quick transport without releasing incisions is very risky. It is a
procedure that should be known to medical rescuers, at least at the theoretical level, and
considered life-saving, even if they do not have sufficient competences to perform it.
Respondents showed interest and effort for deepening of their knowledge about this
procedure during the research.
This situation may be improved by additional training in traumatology, surgery and
intensive care not only in medical rescuers working in medical rescue services, but in
students of these specialisations as well. This thesis could be a base for deeper
investigation of this topic, currently insufficiently known; furthermore, for increasing
specialised knowledge of medical rescuers in treatment of oedematous conditions and
increasing of health care quality in prehospital acute care. | eng |
