| dc.description.abstract | Diplomová práce se zabývá úlohou multidisciplinárního týmu v geriatrické paliativě. Cílem je zmapovat jak paliativní geriatrickou péči, tak pozice, úkony a úkoly členů multidisciplinárních týmů, stejně jako jejich provázanost či případné limity v týmové práci. Cílem práce je rovněž nahlédnout geriatrickou paliativní péči optikou koordinované rehabilitace. Práce je rozdělena do dvou základních částí. První je teoretická a popisuji v ní současný stav tématu. Druhá část práce se zaměřuje na výzkumný cíl. Stanovuji výzkumné otázky, popisuji použitou metodiku výzkumu, předkládám vlastní výzkum a jeho výsledky. Práce má svou diskuzi i závěr. Výsledky mohou čtenářům poskytnout možnost zorientovat se v úloze multidisciplinárního týmu v geriatrické paliativní péči, a nahlédnout ji v kontextu ucelené (koordinované) rehabilitace. | cze |

| dc.description.abstract-translated | My thesis focuses on the role of a multidisciplinary team in the context of the coordinated rehabilitation with the special emphasis on the teamwork in the geriatric palliative care. The aim of the thesis is to map palliative the geriatric care and the roles, activities and tasks of members of multidisciplinary teams. It should also map their interconnection and possible limits in their teamwork. Another aim of my paper is to look at the geriatric palliative care using the perspective of the coordinated rehabilitation. The work consists of two main parts. The first part is theoretical and describes the current state of the topic. The second part focuses on the research goal of the work. The research questions are outlined, the methodology described, and the research itself is presented, including its results. The thesis also includes discussion and conclusion. The results of my thesis can provide readers with the opportunity to get an insight into the work of multidisciplinary teams in the geriatric palliative care, in the context of the comprehensive (coordinated) rehabilitation. | eng |
