| dc.description.abstract | Předmětem této práce je multidiscipinární spolupráce ve službě rané péče. Teoretická část charakterizuje ranou péči a její cílovou skupinu. Dále popisuje složení multidisciplinárního týmu a zabývá se koordinovaným systémem rehabilitace. Praktická část je zaměřena na fungování multidisciplinární spolupráce v konkrétním zařízení, v centru Arpida. Tato část práce obsahuje kvalitativní výzkum, který byl zrealizován pomocí rozhovorů s poradkyní rané péče a vedoucí sociální pracovnicí. V těchto rozhovorech se zabývám projektem "Kruh podpory", který je zaměřen na práci s rodinou pomocí nových metod a nástrojů. Dále se zabývám historií a strukturou multidisciplinárního týmu v centru Arpida a činnostmi služby rané péče. | cze |

| dc.description.abstract-translated | The subject of this work is a multidisciplinary cooperation in early care. The theoretical part characterizes early care and it's target group. Furthermore, it describes the composition of a multidisciplinary team and deals with a coordinated system of rehabilitation. The practical part is focused on the functioning of multidisciplinary cooperation in a specific facility, Arpida center. This part of the work contains qualitative research, which was carried out through interview with an early care counselor and a leading social worker. In these interviews, I deal with a project "Circle of support" which is focused on working with the family using new methods and tools.
